INSURANCE
MEDICAL INSURANCE
- Must work 30 hours or more per week to be eligible.
- Coverage is effective on date of hire. Participation is voluntary.
- PPO in network coverage.
- High Deductible Health Plan with Health Savings Account.
DENTAL INSURANCE
- The dental care plan requires same eligibility as medical insurance (a separate deductible applies).
- Participation is elected separately from medical and vision enrollment.
VISION CARE PLAN
- The vision care plan requires same eligibility as medical insurance (a separate deductible applies).
- Participation is elected separately from medical and dental enrollment.
OTHER BENEFITS AND SERVICES
HEALTH SAVINGS ACCOUNT
- The Health Savings Account (HSA) allows pre-tax dollars to pay for any eligible out-of-pocket health care costs.
- Participation in an HSA is dependent on enrollment in medical insurance.
- Employer contributions occur twice per calendar year.
LIFE INSURANCE
Must work 30 hours or more per week to be eligible. Coverage begins based on eligibility requirements. Premium paid by Trinity. Supplemental insurance (premium paid by the employee) is offered after obtaining eligibility and each year during open enrollment.
DISABILITY INSURANCE
Must work 30 hours or more per week to be eligible. Coverage begins based on eligibility requirements. Plan provides monthly disability benefits following six months of continuous total disability. Premium paid by Trinity. Supplement insurance (premium paid by the employee) is offered after obtaining eligibility and each year during open enrollment.
DEFINED CONTRIBUTION RETIREMENT PLAN
The Plan is available to employees who meet the eligibility requirements. Trinity provides Safe Harbor Contribution of 2% and Discretionary Profit Sharing of 3%.
PAYDAY
Payday is the 15th and last working day of each month. Automatic direct deposit of paychecks is required.
HOLIDAYS
Trinity provides 11 paid holidays for employees working 30 hour or more per week.
PAID TIME OFF
Must work 30 hour or more per week to be eligible based on length of service. Employees have the option to purchase an additional week of PTO each December for the following calendar year.
